The Atg8 family of ubiquitin-like proteins play pivotal roles in autophagy and other processes involving vesicle fusion and transport where the lysosome/vacuole is the end station. Nuclear roles of Atg8 proteins are also emerging. Here, we review the structural and functional features of Atg8 family proteins and their protein-protein interaction modes in model organisms such as yeast, Arabidopsis, C. elegans and Drosophila to humans. Although varying in number of homologs, from one in yeast to seven in humans, and more than ten in some plants, there is a strong evolutionary conservation of structural features and interaction modes. The most prominent interaction mode is between the LC3 interacting region (LIR), also called Atg8 interacting motif (AIM), binding to the LIR docking site (LDS) in Atg8 homologs. There are variants of these motifs like “half-LIRs” and helical LIRs. We discuss details of the binding modes and how selectivity is achieved as well as the role of multivalent LIR-LDS interactions in selective autophagy. A number of LIR-LDS interactions are known to be regulated by phosphorylation. New methods to predict LIR motifs in proteins have emerged that will aid in discovery and analyses. There are also other interaction surfaces than the LDS becoming known where we presently lack detailed structural information, like the N-terminal arm region and the UIM-docking site (UDS). More interaction modes are likely to be discovered in future studies.

More than half of the population with HIV in the world live in sub-Saharan Africa. HIV-associated chronic lung disease is the most prevalent comorbidity among children and adolescents with HIV in this region. The pathogenesis is multifactorial and thought to be a result of chronic immune activation and repeated lung injury. CD4+ T-cells are the main target for HIV infection. As the majority of CD4+ T-cells reside in the gut, HIV can cause a disrupted gut barrier and gut microbial dysbiosis, contributing to maintain systemic immune activation. We used data from a randomized, placebo-controlled trial of 347 children and adolescents with HIV-associated chronic lung disease in Zimbabwe and Malawi, investigating the effect of azithromycin on lung function. Rectal swabs were collected at inclusion, after 48 weeks of treatment and 6 months after cessation of study drug. 16S rRNA sequencing was performed to assess the gut microbiota. The overall aim of this thesis was to investigate the the gut microbiota of children and adolescents with HIV, and to study the effects of antibiotic (azithromycin) treatment on gut microbial composition and on local lung inflammation. We found that participants with HIV had a reduced gut microbial diversity compared to participants without HIV. Diversity improved with longer time on antiretroviral therapy. Azithromycin reduced the overall gut microbial diversity, but these changes did not persist 6 months after cessation of antibiotics. Exhaled nitric oxide, a measure of local inflammation in the lungs, was associated with acute respiratory exacerbations and biomarkers of inflammation. We found no effect of azithromycin on exhaled nitric oxide. This work highlights the association between HIV and gut microbial dysbiosis and the effect of long-term antibiotics on lung inflammation and composition of gut microbiota. It adds knowledge to the field of HIV-associated comorbidities and can be helpful when designing new studies on gut microbiota and potential microbiota-related targets for interventions in the future.; Over halvparten av de som lever med HIV i verden bor i Afrika, sør for Sahara.
BACKGROUND - The accuracy of estimation of kidney function with the use of routine metabolic tests, such as measurement of the serum creatinine level, has been controversial. The European Kidney Function Consortium (EKFC) developed a creatinine-based equation (EKFC eGFRcr) to estimate the glomerular filtration rate (GFR) with a rescaled serum creatinine level (i.e., the serum creatinine level is divided by the median serum creatinine level among healthy persons to control for variation related to differences in age, sex, or race). Whether a cystatin C–based EKFC equation would increase the accuracy of estimated GFR is unknown.

METHODS - We used data from patients in Sweden to estimate the rescaling factor for the cystatin C level in adults. We then replaced rescaled serum creatinine in the EKFC eGFRcr equation with rescaled cystatin C, and we validated the resulting EKFC eGFRcys equation in cohorts of White patients and Black patients in Europe, the United States, and Africa, according to measured GFR, levels of serum creatinine and cystatin C, age, and sex.

RESULTS - On the basis of data from 227,643 patients in Sweden, the rescaling factor for cystatin C was estimated at 0.83 for men and women younger than 50 years of age and 0.83+0.005×(age–50) for those 50 years of age or older. The EKFC eGFRcys equation was unbiased, had accuracy that was similar to that of the EKFC eGFRcr equation in both White patients and Black patients (11,231 patients from Europe, 1093 from the United States, and 508 from Africa), and was more accurate than the Chronic Kidney Disease Epidemiology Collaboration eGFRcys equation recommended by Kidney Disease: Improving Global Outcomes. The arithmetic mean of EKFC eGFRcr and EKFC eGFRcys further improved the accuracy of estimated GFR over estimates from either biomarker equation alone.

CONCLUSIONS - The EKFC eGFRcys equation had the same mathematical form as the EKFC eGFRcr equation, but it had a scaling factor for cystatin C that did not differ according to race or sex. In cohorts from Europe, the United States, and Africa, this equation improved the accuracy of GFR assessment over that of commonly used equations. (Funded by the Swedish Research Council.)

Background and objectives: Chronic headaches are a frequent cause of pain
and disability. The purpose of this randomized trial was to examine whether
transcranial direct current stimulation (tDCS) applied to the primary motor
cortex, reduces pain and increases daily function in individuals suffering from
primary chronic headache.

Materials and methods: A prospective, randomized, controlled trial, where
participants and assessors were blinded, investigated the effect of active tDCS
vs. sham tDCS in chronic headache sufferers. Forty subjects between 18 and
70 years of age, with a diagnosis of primary chronic headache were
randomized to either active tDCS or sham tDCS treatment groups. All patients
received eight treatments over four consecutive weeks. Anodal stimulation
(2 mA) directed at the primary motor cortex (M1), was applied for 30 min in
the active tDCS group. Participants in the sham tDCS group received 30 s of
M1 stimulation at the start and end of the 30-minute procedure; for the
remaining 29 min, they did not receive any stimulation. Outcome measures
based on data collected at baseline, after eight treatments and three months
later included changes in daily function, pain levels, and medication.

Results: Significant improvements in both daily function and pain levels were
observed in participants treated with active tDCS, compared to sham tDCS. Effects
lasted up to 12 weeks post-treatment. Medication use remained unchanged in
both groups throughout the trial with no serious adverse effects reported.

Conclusion: These results suggest that tDCS has the potential to improve daily
function and reduce pain in patients suffering from chronic headaches. Larger
randomized, controlled trials are needed to confirm these findings.

Background: Despite Norway's high living standards and robust welfare system, a paradox exists concerning its young working-age adults - low unemployment but high and increasing uptake of health-related welfare benefits, mainly due to mental health problems. My study investigates the relationship between Norway's welfare system, young adults' reliance on health-related benefits, social health determinants, mental health, employment, and vocational rehabilitation, particularly the evidenced-based Individual Placement and Support (IPS) approach. It reflects on three decades of economic changes, the consequences of welfare reform and the effect of IPS implemented as a cross-sectoral collaboration at the municipality level.

Aims: My overarching aim is to investigate the different factors influencing young adults' transitions into health-related welfare dependency, the effect of major welfare reform on the uptake of health-related benefits, and the societal impact of IPS implementation on competitive employment. It seeks to inform policy-making better to serve young adults in Norway.

Method: Utilising Norwegian register data I conducted longitudinal, comparative analyses across multiple cohorts. Sequence and cluster analyses identify typical health-related benefit trajectories among young adults in Norway. A difference-in-differences method estimates the causal effect of IPS implementation on employment (work days per year) among young adults on a health-related rehabilitation benefit, the Work Assessment Allowance (WAA).

Findings: Most young adults receiving health-related benefits are early school leavers with weak labour market attachment. After the welfare reform, this group saw a substantial rise in long-term health-related benefit dependency. IPS had a significant positive effect on the number of days young adults receiving the WAA worked per year.

Conclusion(s): Current Norwegian policy does not adequately address the needs of young adults with low educational attainment, weak labour market attachment and mental health problems. There is a need for a shift in policy towards cross-sectoral collaboration, integrating evidenced-based vocational rehabilitation approaches such as IPS.;

Purpose: We investigated the association between self-rated health (SRH) and cancer incidence and SRH and all-cause mortality among Norwegian women.

Population and Methods: We used data from 110,104 women in the Norwegian Women and Cancer (NOWAC) cohort aged 41– 70 years at baseline. We used flexible parametric survival analysis with restricted cubic splines to calculate hazard ratios (HRs) and 95% confidence intervals (CIs) for the association between SRH and mortality in the entire cohort. We employed the same method in a multistate design to assess associations between baseline SRH and 1) cancer incidence, and 2) all-cause mortality in subgroups of women who did and did not receive a cancer diagnosis during follow-up.

Results: With very good SRH as reference category for all associations and median age at end of follow-up, lower SRH was associated with increased mortality (HRgood SRH 1.19, 95% CI 1.12– 1.26) and HRpoor SRH 1.81, 95% CI 1.66– 1.97). Lower SRH at baseline was associated with cancer incidence (HRgood SRH 1.14, 95% CI 1.08– 1.20 and HRpoor SRH 1.44, 95% CI: 1.32– 1.58). Poor baseline SRH was associated with increased mortality for women who received a cancer diagnosis (HRpoor SRH 1.20, 95% CI 1.04– 1.39), and SRH showed a strong association with increased mortality for women who stayed cancer free (HRgood SRH 1.59, 95% CI 1.44– 1.77 and HRpoor SRH 3.34, 95% CI 2.91– 3.84).

Conclusion: Lower SRH at baseline predicted increased cancer risk and all-cause mortality in middle-aged to older women. Poor SRH at baseline predicted all-cause mortality in women who later received a cancer diagnosis. Both good and poor SRH at baseline predicted all-cause mortality in women who stayed cancer-free, and the association was stronger for these women compared to both the entire cohort and to women who were subsequently diagnosed with cancer.
