Abstrakt
Tittel: Jordmødres bruk av fødestillinger i forløsning av barnet og sammenhengen med egen preferanse, kvinners brukermedvirkning og rammefaktorer relatert til klinisk praksis.
Hensikt: Hensikten med studien er gjennom å kartlegge selvrapporterte fødestillinger jordmor har brukt siste året, å undersøke hvilke faktorer som påvirker bruk av fødestilling.
Metode: Tverrsnittstudie med spørreundersøkelse på nett. Dataanalyser: deskriptiv statistikk,
kji-kvadrat, Spearmans korrelasjon og Kruskal-Wallis med Mann-Whitney U som
post-hoc test.
Utvalg: 247 jordmødre fra de fire helseforetakene i Norge deltok. Utvalget var rekruttert fra den lukkede facebook-gruppen “jordmødre i Norge” og gjennom privat bedrifts e-post ved 43 fødeenheter.
Resultat: Jordmødrene rapporterte en variert bruk av ni fødestillinger. De forløste hyppigere i liggende fødestillinger sammenlignet med oppreiste fødestillinger. Det var sterk sammenheng mellom bruk av samtlige fødestillinger og jordmødrenes preferanser. Redd for rift/vanskelig å støtte og lite erfaring var hyppig oppgitt som årsak for redusert preferanse for de oppreiste fødestillingene. Jordmødrene oppga at de i høy grad både informerer fødekvinnene og lar fødekvinnen velge fødestilling. Rammefaktorer knyttet til klinisk praksis hadde noe
sammenheng med bruk av enkelte fødestillinger.
Konklusjon: Bruk av fødestilling er påvirket av jordmors preferanse, og ulike faktorer knyttet til jordmors praksis. Sammenhengene kan muligens assosieres med informasjon, utdanning, retningslinjer, erfaring og kultur. Resultater må tolkes med forsiktighet i lys av design og analysemetode. Studien angir ingen årsaksforklaring, og beskriver kun bivariate sammenhenger.

More than half of the population with HIV in the world live in sub-Saharan Africa. HIV-associated chronic lung disease is the most prevalent comorbidity among children and adolescents with HIV in this region. The pathogenesis is multifactorial and thought to be a result of chronic immune activation and repeated lung injury. CD4+ T-cells are the main target for HIV infection. As the majority of CD4+ T-cells reside in the gut, HIV can cause a disrupted gut barrier and gut microbial dysbiosis, contributing to maintain systemic immune activation. We used data from a randomized, placebo-controlled trial of 347 children and adolescents with HIV-associated chronic lung disease in Zimbabwe and Malawi, investigating the effect of azithromycin on lung function. Rectal swabs were collected at inclusion, after 48 weeks of treatment and 6 months after cessation of study drug. 16S rRNA sequencing was performed to assess the gut microbiota. The overall aim of this thesis was to investigate the the gut microbiota of children and adolescents with HIV, and to study the effects of antibiotic (azithromycin) treatment on gut microbial composition and on local lung inflammation. We found that participants with HIV had a reduced gut microbial diversity compared to participants without HIV. Diversity improved with longer time on antiretroviral therapy. Azithromycin reduced the overall gut microbial diversity, but these changes did not persist 6 months after cessation of antibiotics. Exhaled nitric oxide, a measure of local inflammation in the lungs, was associated with acute respiratory exacerbations and biomarkers of inflammation. We found no effect of azithromycin on exhaled nitric oxide. This work highlights the association between HIV and gut microbial dysbiosis and the effect of long-term antibiotics on lung inflammation and composition of gut microbiota. It adds knowledge to the field of HIV-associated comorbidities and can be helpful when designing new studies on gut microbiota and potential microbiota-related targets for interventions in the future.; Over halvparten av de som lever med HIV i verden bor i Afrika, sør for Sahara. Hensikt: Å utforske norske jordmødres erfaringer i møte med innvandrerkvinner i fødsel som ikke behersker det norske språk.
Metode: Kvalitativ metode med åtte individuelle intervju. I dette studiet ble det brukt en hermeneutisk tilnærming og data ble analysert etter livsverden hermeneutikk.
Funn: Funnene presenteres gjennom fem temaer: språkbarrierer kan gi disharmoni og hindre deltakelse, språkbarrierer kan føre til jordmordominans og dårligere omsorg, jordmødrene streber etter harmoni og å være verge, medikalisert fødsel på grunn av språkbarrierer og disharmoni kan føre til grenseoverskridelse. Hovedtolkningen er basert på underbegrepene fra teorien «Birth territory: A theory for midwifery practice» av Fahy og Parrat (2006): integrerende makt, ikke-integrerende makt, jordmorformynderskap og jordmordominans. Hovedtolkningen fra studiet viser at i møte med innvandrerkvinner i fødsel som ikke behersker norsk, er det ikke-integrerende makt og jordmordominans som er fremtredende. Dersom jordmor ønsker å være en vokter og anvende integrerende makt, er endringer nødvendig i praksis slik at jordmødre kan styrke innvandrerkvinnene til å kunne ta makten tilbake i føderommet.
Konklusjon: For at jordmødre skal kunne møte innvandrerkvinner som ikke behersker språket og deres behov under fødsel er det mange utfordringer i omsorgen som må løses. Dagens praksis etterlater jordmødre med psykiske belastninger som fører til at jordmødre krysser egne og innvandrerkvinners grenser. Jordmødrene behøver strategier for å kunne forbedre kommunikasjon som involverer kvinnen og som unngår en medikalisert fødsel. BACKGROUND - The accuracy of estimation of kidney function with the use of routine metabolic tests, such as measurement of the serum creatinine level, has been controversial. The European Kidney Function Consortium (EKFC) developed a creatinine-based equation (EKFC eGFRcr) to estimate the glomerular filtration rate (GFR) with a rescaled serum creatinine level (i.e., the serum creatinine level is divided by the median serum creatinine level among healthy persons to control for variation related to differences in age, sex, or race). Whether a cystatin C–based EKFC equation would increase the accuracy of estimated GFR is unknown.<p>
<p>METHODS - We used data from patients in Sweden to estimate the rescaling factor for the cystatin C level in adults. We then replaced rescaled serum creatinine in the EKFC eGFRcr equation with rescaled cystatin C, and we validated the resulting EKFC eGFRcys equation in cohorts of White patients and Black patients in Europe, the United States, and Africa, according to measured GFR, levels of serum creatinine and cystatin C, age, and sex.<p>
<p>RESULTS - On the basis of data from 227,643 patients in Sweden, the rescaling factor for cystatin C was estimated at 0.83 for men and women younger than 50 years of age and 0.83+0.005×(age–50) for those 50 years of age or older. The EKFC eGFRcys equation was unbiased, had accuracy that was similar to that of the EKFC eGFRcr equation in both White patients and Black patients (11,231 patients from Europe, 1093 from the United States, and 508 from Africa), and was more accurate than the Chronic Kidney Disease Epidemiology Collaboration eGFRcys equation recommended by Kidney Disease: Improving Global Outcomes. The arithmetic mean of EKFC eGFRcr and EKFC eGFRcys further improved the accuracy of estimated GFR over estimates from either biomarker equation alone.<p>
<p>CONCLUSIONS - The EKFC eGFRcys equation had the same mathematical form as the EKFC eGFRcr equation, but it had a scaling factor for cystatin C that did not differ according to race or sex. Background and objectives: Chronic headaches are a frequent cause of pain
and disability. The purpose of this randomized trial was to examine whether
transcranial direct current stimulation (tDCS) applied to the primary motor
cortex, reduces pain and increases daily function in individuals suffering from
primary chronic headache.<p>
<p>Materials and methods: A prospective, randomized, controlled trial, where
participants and assessors were blinded, investigated the effect of active tDCS
vs. sham tDCS in chronic headache sufferers. Forty subjects between 18 and
70 years of age, with a diagnosis of primary chronic headache were
randomized to either active tDCS or sham tDCS treatment groups. All patients
received eight treatments over four consecutive weeks. Anodal stimulation
(2 mA) directed at the primary motor cortex (M1), was applied for 30 min in
the active tDCS group. Participants in the sham tDCS group received 30 s of
M1 stimulation at the start and end of the 30-minute procedure; for the
remaining 29 min, they did not receive any stimulation. Outcome measures
based on data collected at baseline, after eight treatments and three months
later included changes in daily function, pain levels, and medication.
<p>Results: Significant improvements in both daily function and pain levels were
observed in participants treated with active tDCS, compared to sham tDCS. Effects
lasted up to 12 weeks post-treatment. Medication use remained unchanged in
both groups throughout the trial with no serious adverse effects reported.
<p>Conclusion: These results suggest that tDCS has the potential to improve daily
function and reduce pain in patients suffering from chronic headaches. Larger
randomized, controlled trials are needed to confirm these findings.<br />

<p><i>Background:</i> Despite Norway's high living standards and robust welfare system, a paradox exists concerning its young working-age adults - low unemployment but high and increasing uptake of health-related welfare benefits, mainly due to mental health problems. My study investigates the relationship between Norway's welfare system, young adults' reliance on health-related benefits, social health determinants, mental health, employment, and vocational rehabilitation, particularly the evidenced-based Individual Placement and Support (IPS) approach. It reflects on three decades of economic changes, the consequences of welfare reform and the effect of IPS implemented as a cross-sectoral collaboration at the municipality level.
<p><i>Aims:</i> My overarching aim is to investigate the different factors influencing young adults' transitions into health-related welfare dependency, the effect of major welfare reform on the uptake of health-related benefits, and the societal impact of IPS implementation on competitive employment. It seeks to inform policy-making better to serve young adults in Norway.
<p><i>Method:</i> Utilising Norwegian register data I conducted longitudinal, comparative analyses across multiple cohorts. Sequence and cluster analyses identify typical health-related benefit trajectories among young adults in Norway. A difference-in-differences method estimates the causal effect of IPS implementation on employment (work days per year) among young adults on a health-related rehabilitation benefit, the Work Assessment Allowance (WAA).
<p><i>Findings:</i> Most young adults receiving health-related benefits are early school leavers with weak labour market attachment. After the welfare reform, this group saw a substantial rise in long-term health-related benefit dependency. IPS had a significant positive effect on the number of days young adults receiving the WAA worked per year.
<p><i>Conclusion(s):</i> Current Norwegian policy does not adequately address the needs of young adults with low educational attainment, weak labour market attachment and mental health problems. Purpose: We investigated the association between self-rated health (SRH) and cancer incidence and SRH and all-cause mortality among Norwegian women.<p>
<p>Population and Methods: We used data from 110,104 women in the Norwegian Women and Cancer (NOWAC) cohort aged 41– 70 years at baseline. We used flexible parametric survival analysis with restricted cubic splines to calculate hazard ratios (HRs) and 95% confidence intervals (CIs) for the association between SRH and mortality in the entire cohort. We employed the same method in a multistate design to assess associations between baseline SRH and 1) cancer incidence, and 2) all-cause mortality in subgroups of women who did and did not receive a cancer diagnosis during follow-up.<p>
<p>Results: With very good SRH as reference category for all associations and median age at end of follow-up, lower SRH was associated with increased mortality (HR<sub>good SRH</sub> 1.19, 95% CI 1.12– 1.26) and HR<sub>poor SRH</sub> 1.81, 95% CI 1.66– 1.97). Lower SRH at baseline was associated with cancer incidence (HR<sub>good SRH</sub> 1.14, 95% CI 1.08– 1.20 and HR<sub>poor SRH</sub> 1.44, 95% CI: 1.32– 1.58). Poor baseline SRH was associated with increased mortality for women who received a cancer diagnosis (HRpoor SRH 1.20, 95% CI 1.04– 1.39), and SRH showed a strong association with increased mortality for women who stayed cancer free (HR<sub>good SRH</sub> 1.59, 95% CI 1.44– 1.77 and HR<sub>poor SRH</sub> 3.34, 95% CI 2.91– 3.84).<p>
<p>Conclusion: Lower SRH at baseline predicted increased cancer risk and all-cause mortality in middle-aged to older women. Poor SRH at baseline predicted all-cause mortality in women who later received a cancer diagnosis. Both good and poor SRH at baseline predicted all-cause mortality in women who stayed cancer-free, and the association was stronger for these women compared to both the entire cohort and to women who were subsequently diagnosed with cancer.<br />

Bakgrunn: Helsetjenestens organisering med en inndeling i spesialist- og kommunehelsetjeneste krever i stor grad samhandling på tvers av sektorene. Fysioterapeuters praktiske tilnærming til faget kan skape utfordringer for samhandling på tvers av organisatoriske grenser med ulike kontekstuelle og kulturelle rammer. Hensikten med denne studien er å undersøke fysioterapeuters intraprofesjonelle samhandling på tvers av sektorene. <p>
<p>Metode: Vi gjennomførte kvalitativ metasyntese basert på søk i Oria, Google scholar, NORA og PubMed. Gjennom en systematisk utsiling av kvalitative primærstudier ble 6 studier identifisert som relevant til å kunne besvare problemstillingen. Vi benyttet metaetnografisk analyse for å syntetisere data. <p>
<p>Resultat: Studiene viste at fysioterapeuter i stor grad uttrykte behov for relasjonell samhandling som muliggjorde øving på praktiske ferdigheter og diskusjoner av kliniske utfordringer. Mye av samhandlingen som omtales i litteraturen er av veiledende art hvor fysioterapeuter i spesialisthelsetjenesten forventes å støtte fysioterapeuter i kommunehelsetjenesten. Denne asymmetriske relasjonen ble ansett som potensielt utfordrende for samhandling. Funnene viser riktignok at kommunikasjons- og relasjonsferdigheter kunne bidra til å kompensere for den asymmetriske relasjonen. <p>
<p>Konklusjon: Som et supplement til offentlige føringer som primært peker på organisatoriske løsningsforslag for å imøtekomme samhandlingsutfordringer, viser denne studien at fysioterapeuters relasjonelle kompetanse er av stor betydning for samhandling. The incidence and prevalence of breast cancer (BC) are increasing worldwide. The reasons for this trend are partly unknown. BC has several well documented modifiable and non-modifiable risk factors. The only known, natural protective factors are breastfeeding and parity. The current paradigm is that the protective effect of parity is mediated by permanent genomic changes in a woman’s breast tissue during her first pregnancy. Many gene expression studies examining breast carcinogenesis, have used a variety of non-cancerous, but not truly normal samples of breast tissue as controls. We performed a series of studies with a systems epidemiology approach to explore association between breast cancer and parity in a postmenopausal population. The NOWAC study is a prospective national cohort study of 172 000 Norwegian women included between 1991 and 2007. Data is collected from questionnaires and linked national registries. We found that the women in the Norwegian Women and Cancer Study had a decreasing cumulative incidence rate (CIR) of BC of 0.8% per child, rather than only for the first child. This finding was consistent irrespective of other risk factors and is in line with past studies. We performed two gene expression studies using microarray technology in order to explore the biological processes underlying this protective effect. To ensure the best possible source of normal tissue, we collected breast tissue samples of 400 healthy postmenopausal women from the same NOWAC cohort. Gene expression profiles of 311 of these samples showed no genomic changes associated with parity, although we did find genomic changes associated with obesity, smoking and alcohol. Next, we performed a nested case control study of 311 pairs of the same healthy cohort and women with breast cancer in the NOWAC study. We found no impact of parity on global gene expression levels in either cohort. In conclusion we find that parity is a protective factor for BC for each additional child, not only the first full term pregnancy. Background - The aim was to investigate the distribution of health problems in general practice (GP) among adolescent and young adult (AYA) survivors of lymphoma and to compare problem rates with the general population.<p>
<p>Methods - All GP consultations between 2006 and 2020 were identified from the national GP claims register. Diseases and complaints presented 3–10 years after the cancer diagnosis were identified using the International Classification of Primary Care codes (ICPC-2) and compared by applying logistic regression models, presented with odds ratio (OR) and 95%CI.<p>
<p>Results - A total of 2,500,557 AYAs, of whom 1105 were diagnosed with lymphoma, and over 50 million GP consultations were included. The most common causes of consultation among AYA survivors of lymphoma were related to Hodgkin’s disease (13%), pregnancy (11%), respiratory diseases (9%), psychological problems (8%), musculoskeletal system (6%) and fatigue (4%). Almost all these problems were significantly higher among AYA lymphoma survivors compared to the general population (OR ranging from 1.1 to 1.5).<p>
<p>Conclusion - Compared to the general population, young lymphoma survivors have increased contact with the GP for a considerable number of health problems for up to 10 years post-diagnosis. This emphasizes the importance of robust follow-up and a good flow of information between hospital and primary care.<br />
