dc.description.abstractThe medical case report (CR) is one of the oldest medical genres and it is still popular among clinicians. At the end of the 20th century medical journals gave priority to research articles with more advanced study designs. Following the advent of online publishing, a high number of CRs are now being published. We identify the various merits and limitations of the CR genre. Two important merits are the CRs ability to draw attention to new observations and the ability to present new hypotheses. The CR is also an important tool for teaching and learning clinical medicine. It has limitations, including that it cannot produce epidemiological quantities or allow for causal inferences. In a bibliometric, descriptive study of all medical scientific articles produced in a decade at the University Hospital of North Norway, we found that only 2.2% were CR. In three articles we illustrate the working up and format of modern CRs and demonstrate how CRs can contribute to the scientific literature.en_US
dc.description.abstract<p>Den medisinske kasuistikken er en av de eldste medisinske sjangre. Den er populær blant leserne. I slutten av forrige århundre ble medisinske tidsskrift mindre interessert i å publisere kasuistikker, bl.a. pga. behovet for å avgi plass til forskningsartikler med moderne og mer avanserte forskningsmetoder. Etter at elektronisk publisering ble vanlig har sjangeren fått et oppsving. <p>I avhandlingen drøftes sentrale aspekter ved den moderne kasuistikken sett fra et medisinsk perspektiv og sjangeren ses også i et historisk lys. I den første artikkelen presenteres en gjennomgang av faglitteraturen hvor kasuistikkens fortrinn og begrensninger kartlegges. Blant fortrinnene nevnes bl.a. muligheten til å presentere nye observasjoner/funn og generere hypoteser. Hovedbegrensingene er manglende evne til å presentere epidemiologiske funn, trekke konklusjoner om årsakssammenhenger, og generalisere fra funnene. <p>Den andre artikkelen er en bibliometrisk, deskriptiv studie av kasuistikker som ble publisert av forfattere fra Universitetssykehuset Nord Norge over en 10 års periode (2004–2013). Dataene er hentet fra databasen Cristin.no. Vi fant at bare 2,2% av de vitenskapelige publikasjonene var kasuistikker. Det ble også presentert funn vedrørende forfatterskap, medisinske spesialiteter, om målsettingen var vitenskapelig eller læringsmessig, antall siteringer o.a. <p>En sentral del av avhandlingen omhandler tre artikler i form av kasuistikker. Temaene for disse er: 1. Fysisk holding av pasient med påfølgende hypoksi som kunne endt fatalt. 2. Sannsynlig clozapin-indusert diffus parenchymal lungesykdom og samtidig perimyocarditt. 3. Clenched fist syndrome, en nyere tilstand beskrevet i 1980, der kunnskapen fortsatt er begrenset. De tre artiklene illustrerer trekk ved moderne kasuistikker og representer hver for seg viktige bidrag til medisinsk kunnskap.en_US
