Mennesker med utviklingshemming og psykiske helseutfordringer. En kvalitativ studie av vernepleierens erfaringer i å jobbe med mennesker med utviklingshemming og psykiske helseutfordringer

Abstract

Bakgrunn: Tidligere var det lite kjent at personer med utviklingshemming kunne utvikle psykiske helseutfordringer. I en årrekke har de vært diagnostisk overskygget der kognitive utfordringer har overskygget symptomer på psykiske lidelser. I dag vet vi at personer med utviklingshemming har høyre forekomst av psykiske helseutfordringer enn befolkningen for øvrig og at de har høyrere risiko for å utvikle det. Tidligere forskning viser at mennesker med utviklingshemning er underrepresentert ved utredning, behandling og fagoppmerksomhet innen psykisk helsevern. Personer med utviklingshemming mottar i stor grad kommunale tjenester med bistand fra helsepersonell. Det er derfor helt sentralt å sette fokus på tema, i denne sammenhengen gjennom å undersøke hvilke erfaringer helsepersonell har når det gjelder utviklingshemming og psykiske helseutfordringer.

Hensikt: Å undersøke og få innsikt i hvilke mulige utfordringer vernepleiere møter på i arbeid med mennesker med utviklingshemming og samtidig psykiske helseutfordringer.

Problemstilling: «Hvilke erfaringer og refleksjoner har vernepleiere ved omsorgsbolig når det gjelder å jobbe med personer med utviklingshemning og psykiske helseutfordringer?»

Metode/design: Studien har et kvalitativt eksplorativt design, med en fenomenologisk hermeneutisk tilnærming. Data er samlet inn gjennom semistrukturerte intervju med syv vernepleiere fra to kommuner og analysert ved bruk av systematisk tekstkondensering (STC).

Resultat og konklusjon: Resultatene peker på at det er udekkede behov hos de ansatte, og at det er behov for mer støtte og veiledning fra spesialisthelsetjeneste. Det er utfordrende skille mellom hva som er utfordringer knyttet til diagnosen utviklingshemming og hva som skyldes psykiske helseutfordringer. Resultatene peker på at det er usikkerhet både hos ansatte og samarbeidspartnere når det gjelder hvordan man kan møte utfordringer knyttet til psykiske helseutfordringer. Det er behov for mer kompetanse om hva psykiske helseutfordringer innebærer for mennesker med utviklingshemming. Resultatene peker på at mennesker med utviklingshemming kan ha økt behov for tilrettelegging, stabilitet og kontinuitet i hverdagen.

Abstract Background: There have been few studies conducted in regards to researching whereas people with developmental disabilities could develop mental health challenges. For several years, this group of people has been diagnostically overshadowed, as cognitive challenges have overshadowed symptoms of mental disorders. Today, we know that people with developmental disabilities have a higher incidence of mental health challenges than the general population, as well as a higher risk of developing mental health challenges. Previous research and findings indicate that people with mental disabilities are underrepresented in estimation, treatment and professional attention in mental health care. People with mental illness and disorders are entitled to comprehensive, accessible and individually tailored services. Therefore, it is important to focus on said topic; in this context by examining what experiences health professionals have when it comes to developmental disabilities and mental health challenges. Purpose: To investigate and gain insight into the possible challenges that social workers face in working with people with developmental disabilities, who are struggling with mental health challenges. Research question: "What experiences and reflections do social workers have in care facilities when it comes to working with people with developmental disabilities and mental health challenges?" Method/design: The study has a qualitative exploratory design, with a phenomenological hermeneutic approach. Data were collected through semi-structured interviews with seven social workers from two counties and were analysed using systematic text condensation (STC). Results and conclusion: The results indicate that there is an unmet need among the employees and that there is a need for more support and guidance from specialist health services. It is difficult to distinguish between what challenges are associated with the diagnosis of developmental disability and what may be due to mental health challenges. The employees experience a lack of focus on, as well as knowledge concerning the said topic, and they generally describe uncertainty both among employees, yet also among the collaborators when it comes to how to take on these challenges. There is a need for more competence about what mental health challenges entail for people with developmental disabilities. The results indicate that people with developmental disabilities may have an increased need for facilitation, stability and continuity in everyday life.
