Formålet med denne oppgaven er å fremstille en oversikt over de vesentlige gjennombruddene i det kardiologiske fagfeltet. Jeg ønsker å beskrive kort hvordan kunnskapen om hjertet har utviklet seg frem til i dag med fokus på anatomi og basal funksjon.. Hovedtemaet vil være utviklingen av den perkutane coronare intervensjonen, PCI, og hvordan den har endret behandlingen for akutt hjerteinfarkt. Hvordan startet dette kliniske feltet, veien frem til i dag, og i hvilken retning ser det ut til at forskningen på området går videre. PCI har blitt den foretrukne revaskulariseringsmetode hos de fleste pasienter med koronar sykdom og i 2010 ble det utført 11805 PCI behandlinger i Norge, som nesten en tredobling på 15år (3230 i 1995) og som utgjør 241 behandlinger per 100 000 innbyggere(Melberg 2012). Samtidig har antall dødsfall grunnet akutt hjerteinfarkt falt fra 6633 til 3340 i samme periode. (Statistisk sentralbyrå, dødsfall av akutt hjerteinfarkt, alle aldre, alle kjønn, 1995-2010). På tross av en formidabel utvikling i terapeutiske muligheter og med påfølgende fall i mortalitet for akutte hjertesykdommer, er hjertesykdom fremdeles den ledende dødsårsaken i verden
Muligheten for å raskt åpne opp og få tilbake perfusjonen etter at en koronararterie har gått tett, synes per nå, å være det viktigste tiltaket for å bevare mest mulig hjertemuskulatur. Likevel tyder forskningen på at en stor andel av skaden på hjertet etter en okklusjon av et koronarkar, skyldes reperfusjonen. Forebygging av reperfusjonsskader er et felt det forskes mye på i de kardiologiske forskningsmiljøene, spesielt i basalfagene. Både for å forstå mekanismen bak skadene og å forhåpentligvis kunne introdusere en effektiv behandling .
Studien ble gjort som et litteraturstudie fra mars 2015 til og med mai 2014.
