Background: Spinal synovial cysts is an uncommon presentation of spondylosis, but also a cause of considerable morbidity. The condition is surgically treatable, but good-quality data about outcome after treatment is scare.

Material and method: This study is a retrospective analysis of data collected into the NOR-spine, in the period 2007-2020. We included all patients who underwent surgical treatment for spinal synovial cysts, and excluded patients with additional diagnosis such as spinal stenosis and disc herniation. We collected surgeon- and patient reported data at baseline, and patient-reported outcome after 12 month follow-up. The primary aim was disability after surgery, measured in Oswestry Disability Index. Secondary outcomes were leg- and back pain, health quality of life, patients-evaluated benefits and complications after surgery.

Results: A total of 473 patients were included at baseline. At 12 month follow-up, 315 patients remained for analysis. The patients had an increase in Oswestry Disability Index score at 31.2 points (71%).

Conclusion: We concluded that surgical treatment of the cysts shows good outcomes with few complications. Patients reported minimal disability and decrease in both back- and leg pain.en_US
