Epidurals påvirkning på fødselstider og aktiv trykketid

Bakgrunn: Formålet med denne oppgaven var å studere epidural analgesis påvirkning på fødselsforløpet og aktiv trykketid hos fødende.
Material og metode: Data fra Partus ved Fødeavdelingene UNN, Tromsø, Finnsnes og Nordreisa, i tidsperioden 01.01.2007-31.12.2014 inngår i oppgaven. Det ble registrert 12.812 fødsler i denne perioden. Vi inkluderte kvinner som fødte etter 36 svangerskapsuke, hadde ett foster innstilt i normalt hodeleie. Totalt ble 3075 kvinner ekskludert og studiepopulasjonen inkluderer 9737 fødsler. Hovedutfall er aktivtrykketid i minutter og hovedeksposisjon er epidural/ikke-epidural. Statistiske analyser er gjort i SPSS versjon 22.0 med kji-kvadrat test for kategoriske variabler og T-test for gjennomsnittsverdier, mens analyser for forskjeller i median på tvers av grupper er gjort med non-parametrisk test
Resultater
Totalt fikk 22% av kvinnene epidural, signifikant flere førstegangsfødende (33.7%) enn flergangsfødende (12%) (p<.001). På analysene av hele fødselstiden ser vi at gjennomsnittet for hele populsjonen ligger på 345 min. I forhold til epiduralbruk ser vi at førstegangsfødende uten epidural har median lengde på 334 min (5.6t), mens de med epidural har en median tid på 578 min (9.6t). Flergangsfødende uten epidural har median fødselslengde 197 min (3.3t), mens de med epidural hadde median på 408 min (6.8t). For førstegangsfødende var median lengde på aktiv trykketid 34.0 min hos de uten epidural og 40.0 hos de med (p<.001). For flergangsfødende var median lengde på aktiv trykketid 11.0 minutter hos de uten epidural og 19.0 min hos de med epidural (p<.001).
Konklusjon
Epidural gir signifikant lengre fødselsforløp og aktiv trykketid både hos første- og flergangsfødende
